It is vitally important that all workplace accidents AND near misses are reported right away. The forms that need to be filled out can be found by clicking in the following order in Atrieve.

MY INFO > DOCUMENTS eFORMS > MY eFORMS LIST > INCIDENT REPORT > Click on Arrow > INCIDENT REPORTING.

Click on Incident Reporting and the injury form will open up.  All fields that show in yellow are mandatory on the form.
